Doctor Visit Notes and Questions for the Family

At each visit you write only what changed: Started, Dose changed or Stopped. The app goes through the visits from the oldest to the newest and lists what is still being taken, with the date and the doctor. A dose change with no new dose keeps the old dose. It only knows what you wrote, so always check with the doctor or the pharmacy label.

Type a question in the person's box whenever you think of it, at home or in the waiting room. Pasting a list adds one question per line. At the doctor, tap Mark asked, or tick the questions in the visit notes. Asked questions move to "Asked before", where Ask again brings one back.

A visit is one that already happened, with what the doctor said. An appointment is still to come. When you write the notes for an appointment (or add a visit with the same doctor from 3 days before to 14 days after it) the appointment is done and leaves "Coming up". An appointment whose day has passed stays at the top until you add the notes or remove it.

Tap Print, Copy or Email in the person's box. You get the health notes, the medicines now, the next visits, the open questions and every visit, newest first.

Your notes stay in this browser on this device: no account, and your data is never sent anywhere. Once loaded, the app works with no connection. If you clear the browser's data the notes are gone, so save a backup file now and then.
